The True Cost of Quality Healthcare: A Plastic Surgeon's Perspective


​Recently, we encountered a situation that highlighted a common misconception about medical costs. A patient came to our clinic for the removal of an arm swelling. We performed an excision biopsy under local anesthesia, achieving a complete removal of the skin lesion with a well-healing scar. However, during the final follow-up, the patient's guardian mentioned that a relative—a dental specialist in a distant town—had offered to perform the same procedure for 2,000 rupees. Our charge? 14,000 rupees.
This vast difference in pricing naturally caused distress and confusion. But was our fee truly a "rip-off," or is there more to the story? Let's delve into the complexities of medical pricing and the value of specialized care.
The Hidden Costs of Quality CareWhen we provide a medical service, we're not just solving one problem—we're addressing multiple concerns simultaneously. In the case of removing a swelling, here are the key issues we tackle:
  1. Removing the swelling: This is the primary goal, but how it's done matters greatly.
  2. Ensuring a good scar: This is often a primary reason patients choose a plastic surgery clinic. We use specific techniques and materials to minimize scarring:
    • High-quality, delayed absorbable sutures like PDS (Polydioxanone), costing 985 rupees per foil
    • Additional sutures like Poliglecaprone, at 700+ rupees per foil
    • Other materials like Polypropylene
    The cost of consumables alone can exceed 3,000 rupees per procedure.
  3. Preventing recurrence: We use specialized equipment like surgical loupes with illumination, costing over 2 lakhs, to ensure thorough removal and reduce the risk of recurrence.
  4. Ensuring patient safety: We invest in monitors, defibrillators, and electrosurgical units to manage potential complications effectively.
The Value of Expertise and QualityHealthcare isn't just about solving a problem—it's about how well and safely that problem is solved. Consider these points:
  • Expertise: A plastic surgeon's specialized training in wound closure and scar management adds significant value.
  • Equipment: High-quality tools and materials contribute to better outcomes but increase costs.
  • Safety measures: Advanced monitoring and emergency equipment ensure patient safety but add to overhead costs.
Making an Informed DecisionAs a patient, you have choices. Like choosing between a roadside garment and a designer piece in a mall, healthcare options vary in price and quality. Here's what to consider:
  1. Understand your needs: Are you looking for just problem resolution, or do you also want optimal aesthetic results and lower risk of recurrence?
  2. Research thoroughly: Understand what different price points offer in terms of expertise, equipment, and potential outcomes.
  3. Ask questions: Don't hesitate to inquire about the reasons behind the cost of a procedure.
